Massive Damage & Lingering Injury
Lingering injury and massive damage. If you drop below 0 or if you ever sustain a single attack that deals half your total hit points (minimum 50 points of damage) you suffer a lingering injury in addition to the hp loss. In either of these two events you roll on the chart below to determine the effect suffered, an appropriate wound to the damage type of the attack will befall your character. For example a fireball causing a character to drop below 0 and lose and eye might burn the eye leaving it useless while a hammer dealing the same injury might rip the eye out all together.
This rule will be applied with a light touch, it has the possibility to remove limbs and have character altering effects and I’m putting it in for the role play effect not as a combat feature. If a player feels an injury is a major detriment to their character there will always be a way out/an alternative available to them. As a note prosthetics of varying levels of power and tech exist and this is full level range and magic campaign; losing an eye, finger, or even limb is hardly permanent.

|6–8||Moderate scar—wound on face (-1 bonus on Charisma-based skill checks for first scar only, consider subsequent cuts as a major scar)|
|9–10||Major scar—severe wound on face (–1 penalty on Charisma-based skill checks‡)|
|11–14||Loss of finger (for every 3 fingers lost, –1 Dex)|
|15–16||Impressive wound (–1 Con)|
Loss of eye (–4 penalty on all sight-based Perception checks)
|18||Loss of leg (speed reduced to half, cannot charge)|
|19||Loss of hand (cannot use two-handed items†)|
|20||Loss of arm (–1 Str, cannot use two-handed items†)|
